Pre-Register Online

To start the pre-surgical treatment process, we require you to complete your personal and health history information online using One Medical Passport.

Please complete the online forms as soon as your surgery has been scheduled or at least five days prior to your surgery date. This will enable the clinical team to begin organizing and documenting your complete medical history and prepare for your surgery.

Before you get started, be sure to have the following items:

  • Your health insurance information. If you have multiple policies, or are insured under another policy holder, you must have all subscriber information.
  • The names, addresses and phone numbers of your physicians.
  • A list of all medications you are taking, their dosage and frequency.
  • A list of the surgical procedures you have had and the approximate dates.

Be sure to keep your username and password. One Medical Passport can be updates for future procedures to save you time and ensure that your medical records are complete and accurate.


The Ambulatory Care Center participates with all major insurance companies in our region. As a courtesy, we will bill your insurance for your treatment. However, you will be responsible for any deductible or co-payment upon rendering of services. We recommend contacting your insurance company prior to receiving services to understand your coverage and out-of-pocket costs.

In accordance with New Jersey’s Out-of-Network Consumer Protection, Transparency, Cost Containment and Accountability Act, we will always provide information to our patients regarding our facility’s standard charges for items and services provided, in a form that is consistent with federal guidelines.

On behalf of our patients, the Ambulatory Care Center will negotiate with all third party payers regarding out-of-network benefits and price reductions.

All co-payments, co-insurance and deductible are the patients’ responsibility and are due at the time of service.

We accept most major credit cards (Visa, MasterCard and Discover).

Our Billing Process:


For any procedure or surgery performed at ACC, your surgeon’s office will submit a bill/statement for the services provided by the surgeon to your insurance. The billing for the surgical component should be discussed between you and the surgeon. The payment owed to the surgeon is for his/her professional services.


The ACC will begin the billing process with the insurance information you provided. ACC will submit a bill/statement from the surgery center for the surgical/technical services provided by the surgery center.

Facility charges are items or services (supplies, medication, equipment and staff) that are directly related to the surgery. Please contact your insurance carrier to review and understand your yearly benefits.


If you received anesthesia during your procedure, a separate bill will be sent to you from the anesthesia group for providing anesthesia services. Should you have any questions about insurance participation, coverage or payment, please contact them directly using the appropriate information on the correspondence you receive on their behalf.


If your physician orders pre-admission testing, you will be billed separately from the laboratory where you had the testing performed.


If your surgeon removes any tissue for examination, the specimen will be sent to Lab Core/Diannon. You may receive a bill from them for the examination preformed by the Pathologist.

Durable Medical Equipment:

Your surgeon may prescribe equipment to aid in your post-operative recovery. These items, such as braces, ice packs and slings, are not included in your facility charges. Your surgeon will discuss these items with you in advance. If prescribed, you may receive a bill from the service provider.
